New Client Intake Packet
I encourage new clients to complete the necessary paperwork in advance of our first visit. This allows us to make the most of the time we have in your first session.
In addition to the New Client Intake Packet, if you will be utilizing an insurance plan for coverage, you should provide a copy of the front and back of your insurance card along with your packet.
